24 hour cancellation notice is required for appointment and a no-show fee may be charged 100% for the time reserved. Rescheduling with no fee if sick or injury day prior of the appointment.
It is the clients responsibility to inform the therapist all medical conditions, allergies, surgeries, pregnancy or injury, that can affect the massage treatment.
This massage is entirely therapeutic and non-sexual in nature.
I understand the risks associated with massage therapy include, but are not limited to: • Superficial bruising • Short-term muscle soreness • Exacerbation of undiscovered injury I therefore release the individual massage therapist from all liability concerning these injuries that may occur during the massage session.
I understand that it is my responsibility to inform my massage therapist of any discomfort I may feel during the massage session so he may adjust accordingly.
I understand that I or the massage therapist may terminate the session at any time. If arrived late the guest will get partial amount of their time.
To receive this massage, clients are to agree above and hereby waive and release the massage therapist from any past, present and future illness or injury related to the massage therapy and bodywork in case of any accident and all liability. Must fill out and sign intake form before receiving massage.
$20 Birthday discount only valid the week of clients birthday. Must show proof with ID or Driver's license.
